Please note: these are not insurance plans. They are complementary plans designed to provide discounts with in-network providers. They may not cover all benefits such as immunizations, preventative visits, and office visits. Please contact your plan for full disclosure of covered benefits.

Please have your insurance card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If you are not insured, your insurance is inactive at the time of service, or we are not an in-network provider for your insurance, you will be responsible for payment at the time of service.


If you are a new patient, please be sure to call your insurance company and change your PCP to Dr. Diogene prior to your visit. If this is not done prior to the appointment, your appointment may need to be rescheduled.
